Bad Check Notice & Final Notice (2 Forms)
Two letters to use for remittance to the client when receiving bad checks. The first is basically to advise of receiving a bad check and requesting the client make the check good and the second is a final notice indicating if payment is not received forthcoming collection will be pursued.

Credit/Debit Card (3 forms)
Three forms: First form is for client to sign giving authorization to use their credit card and may be produced to vendor where credit card is being used if questioned. Second form is for client to sign giving authorization to use credit card information on behalf of the client (such as online or telephone purchases) if client does not provide you with the actual credit card. Third form is for internal use for recording and retaining credit card information for billing purposes.

This agreement is specific to performing services related to pickup up and delivery of prescriptions. It sets forth a process and terms for pickup/delivery, promised security from the time prescriptions are picked up from the pharmacy and delivered to the client. Proof of Pick-up/Delivery
Use this form when hired to pickup and deliver packages, documents and other items. Record client and delivery information pertinent to the request. Serves as proof of pickup and delivery with a signature required for both. It may be provided to client for their files if necessary (but be sure and keep a copy for your files in case a question arises!)
